Physical Examination

Students registering for Athletics must have a current physical examination form downloaded for Athletic Office review before the student is authorized to participate in tryouts or practice.  Physicals expire 13 months from the date of the physicals.  Physicals forms should be uploaded to the your athletic registration account.
